How to obtain License?

a) Learner's License

Forms Required:

  FORM NO.1 (Medical Certificate from MBBS Doctor) with self photo attested by the doctor with blood Group
  FORM NO.2 (Application form of Learner License) Self Photo

Enclosures:

  Residence Proof (Attested copy of) :

 a. Ration Card OR
 b. Voter’s Identity Card OR
 c. Certified Copy of Voter List OR
 d. Life Insurance Policy at least six months old with Latest Receipt
 e. Valid Passport OR
 f. Employer’s certificate in case of Central Govt./State Govt./Local bodies supported by attested copy of Identity  
 g.Card.

   Age Proof ( Attested copy of ) :

 a. Educational Certificate OR
 b. Birth certificate issued by Registrar Birth and Death showing the name of the applicant OR
 c. Life Insurance Policy at least six months old OR
 d. Valid Passport OR
 e. Employer’s certificate in case of Central Govt./State Govt./Local bodies supported by attested copy of Identity
 f. Card.
 g.Certificate granted by registered medical practitioner, not below the rank of civil surgeon, as to the age of the
 h. applicant.
 i. Driving License issued by RTO/ARTO (for Date of Birth only)

b) Regular License

Documents Required:

 1. Self Photograph
     FORM NO. 4
 2. Valid Learner License

Note : Please bring original documents with photocopies.

c) Duplicate License

Documents Required:

 1. Self Photograph
 2. LLD FORM
 3. Copy of DDR (Showing the license number)
 4. Blood Group in case not mentioned in the last License.

d) Renewal of Driving License

Forms Required:

  FORM NO.9

1. FORM NO.1 (Medical certificate from MBBS Doctor) with blood group

Enclosures:

 1. Residence Proof :

 a. Attested copy Voter card OR
 b. Valid passport OR
 c. LIC cover note at least six months old with latest receipt of payment OR
 d. Employer certificate issued by the competent authority with I-Card in case of employee of State or central
    govt. OR
 e. If an applicant does not have any of the proof mentioned above he may attach copy of Ration Card with
     affidavit

 

Note : Following documents accepted as proof of Address supported by Affidavit attested by Public Notary /Magistrate.

 2. Electricity Charges Bill
